Only nominations that have been duplicated and three-hole punched can be considered.

Teaching Portfolios are required. PLEASE SUBMIT SEVEN COPIES OF YOUR NOMINATION MATERIALS. PLEASE ENSURE THAT THE ORIGINAL DEPARTMENTAL ENDORSEMENT PAGE WITH SIGNATURE IS INCLUDED. THE MATERIALS SHOULD BE THREE-HOLE PUNCHED. PLEASE DO NOT PUT IN A BINDER. PAPER CLIP EACH SET TOGETHER.
